About the Role
As an MDS Coordinator, you will be responsible for completing and managing accurate, timely MDS assessments in compliance with state and federal regulations. You will oversee the RAI process, support interdisciplinary care planning, ensure documentation supports quality outcomes and reimbursement, collaborate with nursing, therapy, and administrative teams, and assist with audits, surveys, and quality improvement initiatives.
About You
Required:
- Active RN or LPN license in good standing.
- Strong knowledge of Medicare/Medicaid guidelines and RAI requirements.
- Excellent organizational skills and attention to detail.
- Ability to work independently and meet deadlines.
Preferred:
- Prior MDS experience in a long-term care setting.
