MDS Coordinator

Outfield Healthcare Partners is seeking an MDS Coordinator to join their team in Lees Summit, MO. The role involves the timely and accurate completion of the RAI process and care management from admission to discharge.

As an MDS Coordinator, you will collaborate with the Interdisciplinary Team to assess resident needs, ensure compliance with federal and state regulations, monitor MDS documentation, and maintain accurate data entry. You will also assist in coordinating meetings, prepare care planning schedules, and ensure the MDS and care plans reflect any significant changes.
