About the Role
About You
- Valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of employment.
- Experience in long-term care or skilled nursing setting.
- Knowledge of MDS 3.0, RAI process, and PDPM.
- Proficiency in electronic health record (EHR) systems and MDS software (e.g., PointClickCare, MatrixCare).
- Strong organizational and time management skills.
- Attention to detail and ability to meet strict deadlines.
- RAC-CT (Resident Assessment Coordinator – Certified) credential through AANAC/AHCA.
- At least 1 year of MDS coordination experience.
- Familiarity with quality initiatives such as QAPI and Five-Star Quality Rating.
