Summary

About the Role

As an MDS Coordinator, you will be responsible for bedside assessments, collecting data from medical records and interviews, and ensuring that the Plan of Care is reviewed and updated quarterly. You will also participate in various meetings to meet federal requirements and educate team members on the MDS process.

About You

Required:

Registered Nurse with required current state licensure.
Minimum three (3) years of clinical experience in a health care setting; long-term care setting preferred.
Minimum one (1) year of MDS experience.

Preferred:

Strong organizational skills and attention to detail.
Ability to work independently and as part of a team.
