About the Role
As a Medical Case Manager, you will manage a caseload of higher-need, high-complexity HIV+ clients, providing medical case management services including coordination of medical services, service advocacy, and resource finding. You will ensure timely access to health and support services, link clients to appropriate services, and provide access to Emergency Financial Assistance as needed.
About You
Required:
- Bachelor's degree in Social Work (BSW) or in a related human service, social science, or health education field with licensure in mental health counseling, or Associate's degree in a related field plus 4 years of case management experience.
Preferred:
- 2 years of experience in community health or with a similar patient population.
- Familiarity with issues affecting individuals living with HIV.
- Intermediate proficiency in Microsoft Office.
- Bilingual fluency (English/Spanish) strongly preferred.
