About the Role
As a Medical Social Worker, you will conduct assessments of new patients, coordinate with interdisciplinary teams for discharge planning, and provide counseling and support to patients and families. You will also document patient information, facilitate conflict resolution, and participate in hospital committees as needed.
About You
Required:
- Master’s Degree in Social Work from an accredited Social Work Program.
- 1-2 years of work experience in a skilled nursing facility, home health, hospice, and/or an acute hospital.
Preferred:
- ASW License.
- Current American Heart Association BLS Certification.
- Hospital experience.
