About the Role
As a Mental Health Case Manager, you will act as a liaison between therapists, referring agencies, and clients' families. Your responsibilities will include coordinating transportation, ensuring ongoing communication, attending court hearings, facilitating treatment staffing, and conducting ongoing assessments of client needs. You will also assist clients in achieving personal independence and facilitate the development of daily living skills, while coordinating individual service plans and providing crisis management support.
About You
Required:
- Bachelor’s degree in Social Work or a related field, or a minimum of two years college credit plus five years of work experience in Human Services.
- Experience working with adolescents, teens, and adults in a mental health context.
- Proven competency with MS Office applications, including Word, Excel, and PowerPoint.
- Ability to work in a challenging and dynamic environment.
Preferred:
- Experience with emotionally disturbed populations.
- Knowledge of Electronic Health Record (EHR) systems.
