Monarch Healthcare Management is seeking a Minimum Data Set (MDS) Coordinator to join their team in St Louis Park, MN. The role involves overseeing the MDS assessment process to ensure accuracy, compliance, and timely completion.
About the Role
As an MDS Coordinator, you will be responsible for editing, locking, and transmitting MDS assessments. Your duties will include performing admission chart reviews, managing Medicare processes, collaborating with the Interdisciplinary Team (IDT) for care plan accuracy, monitoring quality measures, and maintaining compliance with regulations. You will also provide clinical support to nursing staff and participate in quality improvement efforts.
About You
Required:
Must be either a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Minnesota.
Must have successfully completed or seek certification in Cardiopulmonary Resuscitation and emergency care for the health care provider.
Experience in healthcare, skilled nursing facility or assisted living community.
Experience with MDS and Resident Assessment Instrument (RAI).
Preferred:
Ability to communicate verbally and in writing when directing the Resident Assessment Instrument (RAI) process and when performing and assessing care for residents.
