Community Bridges, Inc. is seeking a Navigator I to join their team in Phoenix, AZ. The role involves enhancing client success by connecting them with necessary services and resources while conducting outreach and case management.
About the Role
As a Navigator I, you will work with individuals needing additional support, conducting outreach, engagement, and case management. You will be responsible for mapping out appropriate next steps for clients and documenting required patient interactions according to various regulatory standards. This position is part of a multi-disciplinary team focused on providing comprehensive support to clients experiencing crises, opioid use disorder, homelessness, and mental illness.
About You
Required:
- High school diploma or GED is required.
- Minimum of six months of recovery from substance use and/or mental health disorders required.
- Current AZ Driver's License (valid and good standing).
- Clear 39-month Motor Vehicle Record.
- Arizona Fingerprint Clearance card or good cause exemption required.
Preferred:
- Associates Degree (or higher) in a field related to Behavioral Health.
- 1-3 years of full-time Health Care related work experience.
- 1-3 years of substance abuse/mental health recovery-oriented service.
