Summary

About the Role
As a Navigator II, you will conduct outreach, engagement, case management, and provide ongoing supportive services to individuals in need. You will work as part of a multi-disciplinary team, documenting and tracking patient interactions according to various regulatory standards.
About You
Required:
- High school diploma or GED is required.
- Minimum of six months of recovery from substance use and/or mental health disorders required.
- Minimum of ninety (90) days experience in a Navigator or Peer Support Specialist role required.
- Must possess a current valid driver's license.
Preferred:
- Associate degree (or higher) in a field related to Behavioral Health is preferred.
- 1-3 years of full-time Health Care related work experience is preferred.
- 1-3 years of substance abuse/mental health recovery-oriented service is preferred.
