About the Role
As a Nurse Care Manager, you will address members' clinical, psychosocial, financial, and environmental needs. Your responsibilities include engaging members in self-management, preparing care plans, participating in interdisciplinary meetings, and linking members to community resources. You will also provide education on chronic diseases and preventive care, advocate for members, and maintain accurate documentation.
About You
Required:
- Minimum 2 years care management experience required.
- Bachelor’s Degree required.
- Must possess a current and unrestricted NYS RN License and Registration.
Preferred:
- Medical Surgical Nursing Experience.
- Ability to obtain RN Licenses in CT, PA, and NJ.
