About the Role
As a Nurse Case Manager, you will work with physicians and the Interdisciplinary Team to assess, plan, implement, and evaluate care plans for patients and their caregivers. You will provide case management services, ensure compliance with regulations, document nursing activities, and maintain communication with team members. The role may require on-call duties and travel for patient care.
About You
Required:
- Bachelor's degree in Nursing or related field.
- Current Hawaii license as a Registered Nurse in good standing.
- At least one year of experience in healthcare or gerontology.
- At least two years of experience as a staff nurse in a hospital.
Preferred:
- Experience in hospice and/or home health nursing.
- Current CPR BLS certification.
