About the Role
As a Nurse Practitioner / Physician Assistant, you will conduct comprehensive medical assessments, manage acute and chronic medical and behavioral health conditions, and collaborate with an interdisciplinary care team. You will work a consistent weekday schedule with no on-call, weekends, or holidays, ensuring a true work-life balance while delivering meaningful, relationship-based care.
About You
Required:
- Active, unrestricted NP or PA license in state of residence (or ability to obtain by start date).
- Graduate of accredited NP or PA program.
- National board certification: NP: AANP or ANCC (FNP, ANP, GNP, or AGACNP) or PA: NCCPA.
- Active DEA / prescriptive authority.
- Ability to meet physical requirements (lifting up to 30 lbs, navigating stairs, standing, kneeling).
- Reliable transportation.
- Ability to obtain collaborative practice agreement if required by state.
- Full COVID-19 vaccination (per federal, state, and client requirements).
- Ability to pass pre-employment drug screening.
Preferred:
- 1+ year of clinical experience (long-term care preferred).
- Experience managing patients with complex medical, behavioral, social, or functional needs.
- Knowledge of geriatrics and chronic illness.
- Comfort with advanced illness and end-of-life conversations.
- Proficiency with EMR systems.
