About the Role
As a Nurse Practitioner, you will be responsible for delivering patient-centered care in a home-based setting, focusing on high acuity patients. Your duties will include assessing patient needs, developing care plans, and collaborating with a team of healthcare professionals to ensure quality care. You will work Monday to Friday with minimal call responsibilities and no hospital rounds.
About You
Required:
- Licensed as a Nurse Practitioner in the State where HarmonyCares Medical Group office is located.
- Federal DEA License/eligibility.
- Active CPR/BLS Certification.
- Board Certification.
- Must maintain a valid driver's license and good driving record.
Preferred:
- Geriatric training/experience.
