Hospice of Florida is seeking a Nurse Practitioner (NP or PA) for Palliative Care to join their team in Jacksonville, FL. The role involves delivering expert, compassionate care to patients and families facing serious illness, focusing on quality of life.
About the Role
As a Nurse Practitioner or Physician Assistant in Palliative Care, you will provide holistic assessments, guide patients and families through complex medical decisions, manage symptoms, and ensure care aligns with their goals. You will collaborate with an interdisciplinary team to create personalized care plans, support transitions of care, and document all interactions thoroughly in the EMR.
About You
Required:
Current, unencumbered licensure as a Nurse Practitioner or Physician Assistant in applicable state.
Board Certification in applicable NP or PA specialty.
Master of Science in Nursing (MSN), Doctorate of Nursing Practice (DNP), or Master of Medical Science (MMSc/MPAS).
CPR Certification per state requirements.
Preferred:
Certification in Hospice and Palliative Care preferred (or willingness to obtain within 12 months).
Minimum 1 year of experience in palliative care, geriatrics, or complex chronic disease management preferred.
Strong knowledge of Medicare Part B, palliative care billing, and ICD-10 coding.
Proficiency in EMR and mobile documentation workflows.
Exceptional communication skills (verbal, written, and public speaking).
