About the Role
As a Nurse Practitioner at Strive Health, you will manage patient care by collaborating with a team of social workers, care coordinators, pharmacists, dietitians, and physicians. Your responsibilities include obtaining patient histories, performing physical exams, ordering and interpreting diagnostic tests, and formulating care plans. You will also ensure best practices for chronic kidney care, participate in shared decision-making, and provide care in various settings including telehealth and in-person visits.
About You
Required:
- Master’s degree in Nursing or similar field.
- 2+ years experience as a Family Nurse Practitioner (NP), Primary Care Nurse Practitioner (NP), or experience in internal medicine, cardiology, Nephrology or Case Management.
- Current state specific licensure required.
- National Board Certification.
- Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire.
- Current BLS or CPR Certification required.
- Ability to work Tues-Sat 8a-5p.
Preferred:
- Basic computer and Microsoft Office skills.
- 2+ years experience with Electronic Health Records (EHR) preferred.
- Wound Care certification preferred.
- Experience using audio-visual technology platforms.
