About the Role
As a Nurse Practitioner (Hospice), you will be responsible for evaluating and assessing health histories, diagnosing medical conditions, managing health problems, and coordinating care with patients, families, and healthcare practitioners. You will conduct physical assessments, evaluate hospice eligibility, and collaborate with an interdisciplinary team to ensure quality patient care in compliance with state regulations.
About You
Required:
- Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
- Current nursing licensure in the state and CPR certification.
- Previous home care/hospice experience.
Preferred:
- Current DEA license.
- Accreditation from an approved certifying body for advanced practice nursing.
- Certification in a specialist area such as Hospice and Palliative Nursing (CHPN), Pediatrics, or Geriatrics.
