About the Role
As a Nurse Practitioner or Physician Assistant, you will be part of the Home and Community Care team, responsible for providing comprehensive assessments, developing patient management care plans, and collaborating with interdisciplinary teams to improve patient health outcomes. You will perform nursing procedures, prescribe diagnostics, educate patients, and participate in quality initiatives while ensuring compliance with state laws and regulations.
About You
Required:
- Completion of Nurse Practitioner or Physician Assistant accredited program.
- Current state RN/NP or PA license.
- Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or National Commission on Certification of Physician Assistants (NCCPA).
- Access to reliable transportation.
Preferred:
- 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care.
- Knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems.
- Knowledge of CPT, ICD-10, and HCPC codes.
- Knowledge of clinical standards of care.
- Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems.
