About the Role
As a Nurse Practitioner or Physician Assistant, you will conduct in-home assessments, perform evidence-based screenings, identify diagnoses for care management, and educate patients on health topics. You will work within a flexible schedule, with no on-call, weekends, or holidays required, contributing to a holistic approach to patient care.
About You
Required:
- Active NP license or PA License or ability to obtain by start date.
- Active ANCC or AANP national certification in relevant fields.
- Ability to complete physical requirements of the job including moving a 30-pound bag and navigating various dwelling conditions.
- Driver’s license and access to reliable transportation for home visits.
Preferred:
- 1+ years of clinical experience in a relevant clinical setting.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Home health care or home visit experience.
- Ability to travel throughout the state, beyond assigned region, up to 5% of the time.
- Fluency in additional languages such as Spanish, Cantonese, Korean, Vietnamese, or Polish.
