As a Nurse Practitioner or Physician Assistant, you will conduct in-home assessments, perform evidence-based screenings, identify diagnoses for care management, and educate members on health topics. You will work in a flexible environment with no on-call, weekends, or holidays required, contributing to a holistic approach to patient care.
About You
Required:
- Active NP or PA license or ability to obtain by start date.
- Active ANCC or AANP national certification in relevant fields.
- Ability to complete physical requirements of the job including moving a 30-pound bag and navigating various dwelling conditions.
- Driver's license and access to reliable transportation for home visits.
Preferred:
- 1+ years of clinical experience in a relevant clinical setting.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Home health care or home visit experience.
- Ability to travel up to 5% within licensed states if requested.
