About the Role
As a Nurse Practitioner at Haven Hospice, you will be responsible for providing medical care to patients with terminal illnesses, completing history and physical examinations, assessing hospice eligibility, making recommendations for patient certification, maintaining clinical notes, and ensuring compliance with legal and regulatory requirements. You will also participate in interdisciplinary meetings to address the needs of patients and their families.
About You
Required:
- Graduate from an accredited School of Nursing and accredited Nurse Practitioner program.
- Current nursing license in state of practice.
- Current CPR certification.
- Master’s degree with a minimum of one year NP experience.
Preferred:
- Current DEA license, if applicable to state regulations.
- One year hospice or palliative care experience.
- Accreditation from an approved certifying body for advanced practice nursing.
- Certification in a specialist area (e.g. Hospice and Palliative Nursing (CHPN) or Geriatrics).
