About the Role
As a Nurse Practitioner, you will be responsible for rounding and managing clinical care for patients at assigned skilled nursing facilities (SNFs). Your daily tasks will include prescribing medications, ensuring timely documentation of patient encounters, and being available for consultations with nursing staff. You will work in a supportive community with flexible schedules and no on-call or overnight requirements.
About You
Required:
- Active Nurse Practitioner license in the state of Michigan.
- Experience in providing primary care services.
Preferred:
- Familiarity with electronic health record (EHR) systems such as ChartEasy™, PointClickCare, or MatrixCare.
- Strong communication skills and ability to establish meaningful connections with patients.
