About the Role
As an Outpatient Clinical Pharmacist, you will evaluate, interpret, and manage medications, collaborate with healthcare professionals, and apply evidence-based medication management strategies. Your responsibilities include executing disease state management programs, performing medication reconciliation, educating patients, and maintaining documentation for compliance and billing.
About You
Required:
- Completion of Doctor of Pharmacy (PharmD) degree.
- Active, unrestricted Washington State Pharmacy License.
- Active, unrestricted California State Pharmacy License or willingness to obtain within 1st year of employment.
- 3+ years of experience in clinical pharmacy practice including clinical pharmacotherapies, disease state management and drug use evaluations OR completion of a 1 or 2 year ASHP-accredited residency in Clinical Pharmacy Practice.
Preferred:
- Board certification in pharmacy specialties (i.e. Ambulatory Care-BCACP, Pharmacotherapy-BCPS).
- Active, unrestricted Oregon State Pharmacy License or willingness to obtain.
- Bilingual English/Spanish.
