About the Role
As an Outpatient Social Worker, you will be responsible for delivering integrated care and quality outcomes while collaborating with a multi-disciplinary team. Your daily tasks will include using motivational interviewing, conducting assessments, and implementing clinical interventions to enhance the physical, cognitive, and emotional functioning of patients. You will also participate in quality improvement initiatives and assist members of the accountable care organization in addressing social influencers of health.
About You
Required:
- Master's degree in Social Work is required.
- Three years previous clinical experience in Medical, Psychiatric or Health Care Setting required.
Preferred:
- NYS LMSW or LCSW is preferred.
- Some supervisory experience preferred.
