About the Role
As an Outreach Worker, you will identify and build relationships with high-risk individuals, develop personalized risk reduction plans, intervene in local conflicts, and provide support and resources to clients. You will also conduct community outreach, document activities, and participate in team meetings and community events.
About You
Required:
- High school diploma and a minimum of 2 years' experience in a related area.
- Knowledge of mental health issues and psychosocial triggers.
- Experience working with at-risk youth and gang members.
Preferred:
- Familiarity with Morrisania or Mott Haven neighborhoods.
- Ability to work flexible hours including evenings and weekends.
- Excellent communication skills and commitment to community change.
