About the Role
As a PACE Occupational Therapist, you will provide Occupational Therapy treatment and interventions to participants in various settings, conduct comprehensive assessments, develop personalized care plans, and collaborate with an interdisciplinary team to enhance participants' independence and quality of life. You will also guide participants in therapeutic activities, review progress, and ensure compliance with regulatory requirements.
About You
Required:
- Bachelor's degree from an accredited occupational therapy program and OT licensure.
- Minimum of 3 years of experience in healthcare settings, with at least 1 year working with frail, older adults.
- CPR certification (BLS or ACLS) and a valid driver's license.
Preferred:
- Experience in care management, geriatrics, chronic disease management, home health, or long-term care.
- Proficiency in documenting case notes and assessments accurately.
