About the Role
As a Palliative Care Nurse Practitioner, you will demonstrate effective and ethical use of evidence-based palliative medicine practices, gather pertinent information for comprehensive assessments, and formulate effective care plans. You will also be responsible for medication reconciliation and evaluating patient outcomes, ensuring specialty services for patients who are not ready or eligible for hospice care.
About You
Required:
- RN/APRN licensure in state of practice
- Nurse Practitioner/CNS with prescriptive authority
- BCLS licensure
- Current CPR certification
- 5+ years of clinical nursing experience
- Driver's License and access to reliable transportation
Preferred:
- Advanced practice palliative care certification (ACHPN)
- 1+ years as APRN in Hospice, Palliative Care, or Oncology
