About the Role
As a Palliative Care Nurse Practitioner, you will conduct comprehensive patient assessments, create individualized care plans, and provide ongoing follow-up and chronic care management. You will collaborate with interdisciplinary teams to ensure coordinated care and educate patients, families, and staff on health management. The position offers a Monday–Friday schedule with no nights, weekends, or on-call duties.
About You
Required:
- Active Nurse Practitioner (NP) License in state of practice.
Preferred:
- 2 years of experience preferred but not required.
- Experience in Skilled Nursing Facilities (SNFs), Hospice and/or palliative care, Emergency department, ICU/SICU, or hospitalist settings.
- Proficiency in Electronic Health Records (EHR).
- Strong communication skills and a collaborative, compassionate mindset.
