firsthand is seeking a Part Time Nurse Practitioner to join their team in Norfolk, VA. The role involves providing primary and urgent care for individuals with serious mental illness in various settings.
About the Role
As a Local Health Guide, you will be responsible for conducting in-person assessments, chronic care follow-ups, and urgent care for vulnerable populations. You will collaborate with interdisciplinary teams, support individuals in developing self-management skills, and ensure appropriate clinical documentation. The position requires working at least three days per week in home or office settings.
About You
Required:
- 1+ years clinical care as a Nurse Practitioner, ideally in a home or community primary care setting, or emergency department.
- Ability to prescribe medications for behavioral health conditions including antipsychotics.
- Experience working with populations facing behavioral health and/or substance use disorder challenges.
Preferred:
- Passion for in-person care.
- High comfort level for home and community-based visits.
- Experience working on interdisciplinary teams.
- Excellent listening, communication, and interpersonal skills.
