firsthand is seeking a Part Time Nurse Practitioner to join their team in Richmond, VA. The role involves providing primary and urgent care for individuals with serious mental illness in various settings.
About the Role
As a Local Health Guide, you will be responsible for conducting in-person assessments, chronic care follow-ups, and urgent care for vulnerable populations. You will collaborate with an interdisciplinary team, including behavioral health providers and peer recovery specialists, to create individualized recovery plans and support patients in managing their health conditions.
About You
Required:
- 1+ years clinical care as a Nurse Practitioner, ideally in a home or community primary care setting, or emergency department.
- Ability to prescribe medications for behavioral health conditions including antipsychotics.
- Experience working with and managing a population with challenges such as behavioral health and/or substance use disorders.
Preferred:
- High comfort level for home and community-based visits.
- Experience working on interdisciplinary teams.
- Proficiency with technology for documentation or coordination of care.
