About the Role
As a Patient Assessment Coordinator, you will oversee data collection on the IRF/PAI, ensure compliance with CMS guidelines, and provide staff education. You will review IRF/PAI forms for accuracy, monitor functional status scoring, and coordinate patient information collection. Additional responsibilities include setting completion dates for the multi-disciplinary team and acting as a resource for problem-solving related to Medicare's IRF prospective payment system.
About You
Required:
- Graduate of an accredited OT, PT, SLP, or RN program with current licensure in the state.
- A minimum of three (3) to five (5) years of experience in a patient assessment coordinator role.
Preferred:
- Bachelor's degree preferred for nursing.
- Master's degree required for OT/PT/SLP.
