- Minimum of 2-5 years of experience in healthcare, particularly in patient engagement, case management, or transitional care.
- Strong knowledge of home health regulations and HIPAA compliance.
- Proficiency in healthcare technology systems, such as EHR and CRM tools.
- Excellent communication and interpersonal skills.
