About the Role
As a Per Diem Medication Reconciliation Tech, you will be responsible for collecting patients' medication information from home and current pharmacies, creating and logging complete medication lists, and ensuring accuracy and completeness. You will perform most job duties independently, interact with patients and families, assist in the preparation of intravenous admixture solutions, maintain inventory levels, and ensure cleanliness of pharmacy equipment and work areas.
About You
Required:
- Must be 18 years of age or older.
- Ability to read, write and understand English at the 12th Grade level.
- Must have passed Board of Pharmacy Technician Registration exam.
- Must know basic IV admixture processes and sterile procedures.
- Current PTCB certification or completion of 2 years of pharmacy school.
- Must score 85% or greater on Medication Reconciliation Technician Test.
Preferred:
- Good math, computer, and communication skills.
- Clear understanding of medication names (brand and generic), common dosing and sig codes, and medical terminology.
