About the Role
As a Personal Service Coordinator I/II, you will be responsible for conducting client evaluations, preparing individualized care plans, facilitating access to community resources, and maintaining documentation. You will provide professional support through regular client contact, including home visits, and ensure compliance with program standards. The PSC II role includes additional responsibilities such as conducting evaluations and serving as an advanced resource for case management.
About You
Required:
- Bachelor's degree in Human Services or related field with 1 year of related experience for PSC I.
- Bachelor's degree in Human Services or related field with 4 years of related experience for PSC II.
Preferred:
- Eligible to claim for mental health services (except psychotherapy) within training and scope of practice.
- Up to 2 years graduate education may substitute for experience on a year-for-year basis.
