BAYADA Home Health Care is seeking a Full Time Physical Therapist, Home Health Visits to provide home health visits for their office in Charleston, SC.
About the Role
As a Physical Therapist, you will be an integral member of a multi-disciplinary health care team providing rehabilitative care and skilled nursing to clients. Your responsibilities will include making home visits, performing diagnostic tests, implementing protective interventions, developing individualized care plans, continually assessing and revising care plans, educating clients and families, and accurately documenting client care management.
About You
Required:
- A current SC Physical Therapist - PT license.
- A minimum of one year of recent work experience as a Physical Therapist - PT.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation.
Preferred:
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
