Circle the City is seeking a Physical Therapist PRN Float to join their team in the Greater Phoenix Area. The role involves evaluating and treating patients referred to the Physical Therapy Program by CTC physicians.
About the Role
As a Physical Therapist PRN Float, you will be responsible for conducting complete evaluations of patients, designing individualized treatment plans, documenting each visit in the Electronic Health Record (EHR), and attending weekly multi-disciplinary rounds to discuss patient care plans. You will also complete discharge summaries when treatment goals are met or when patients are discharged according to their wishes.
About You
Required:
- License to practice physical therapy in Arizona.
- Masters of Physical Therapy.
- Basic Life Support certification (BLS).
- Understanding of substance and mental health issues.
- Interpersonal and communication skills to effectively interact with patients, physicians, and health team members.
- Computer proficiency to access, enter, and interpret computerized data/information.
Preferred:
- Demonstrated interest in working with underserved populations.
