About the Role
As a Physical Therapist PT Home Health, you will be responsible for helping patients maintain, restore, and improve their movement, activity, and function. Your daily tasks will include providing interventions, conducting reexaminations every 30 days, modifying interventions as necessary, and performing discharge planning. You will work in a community-based care setting, making a meaningful impact on the lives of patients and their families.
About You
Required:
- Current license to practice physical therapy specific to the state you're assigned to work in.
- Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).
- One year related physical therapy experience for PTs entering home health care.
- Current CPR certification.
Preferred:
- Experience in home health care.
- Approval from regional clinical leaders if less than one year of experience.
