About the Role

You will conduct field visits, document patient progress, and ensure continuity of care while adhering to safety regulations and agency standards.
About You
Required:
- Must have a valid professional license in the state of practice.
- Graduate of an accredited Physical Therapy Program.
Preferred:
- Prior Home Health experience is preferable.
