About the Role
As a Home Health Physical Therapist (PT) Care Manager, you will be responsible for managing a caseload of home health patients, conducting assessments, developing care plans, and ensuring effective communication with patients, families, and healthcare providers. You will evaluate patient progress, adjust care plans as necessary, and document all findings and treatments in clinical records.
About You
Required:
- Master's Degree in Physical Therapy
- 1 year relevant clinical physical therapy experience
- Physical Therapy license in the state of practice
- Valid in-state Driver's License with current car insurance
- Active American Heart Association BLS certification
Preferred:
- Recent experience in a Medicare-certified home health agency as a case-manager
- Certificate for OASIS Specialist – Clinical
- Home Health Case-Manager Certification
