Your Health is seeking a Primary Care Physical Therapist to join their team in Rock Hill, SC. The role involves providing patient care through home visits, clinics, and Telehealth services.
About the Role
As a Primary Care Physical Therapist, you will perform visits in various settings, including homes, assisted living facilities, and clinics. Your responsibilities will include obtaining health histories, conducting physical assessments, providing health education, and coordinating referrals to other healthcare professionals. You will work full-time, Monday to Friday, from 8:00 AM to 5:00 PM, and will be expected to maintain patient confidentiality and comply with legal reporting requirements.
About You
Required:
- Successful completion of a Physical Therapy training program and certification through the therapy state board.
- A current license to practice in the state of SC.
- Current CPR Certificate.
- At least one year of experience in the specialty field.
Preferred:
- Previous experience in a healthcare setting.
- Experience and/or interest in Pelvic Health.
- Ability to work flexible hours and travel between locations.
