As a Physician in Home Based Primary Care, you will be responsible for delivering comprehensive medical care to Veterans in their homes. This includes assessing patient needs, developing care plans, and coordinating with other healthcare professionals to ensure optimal health outcomes. You will focus on geriatric care and work closely with a multidisciplinary team to provide holistic support to patients.
About You
Required:
- Medical degree (MD or DO) from an accredited institution.
- Current medical license to practice in the state of Texas.
- Experience in primary care or geriatrics.
Preferred:
- Board certification in Family Medicine or Internal Medicine.
- Experience with home-based care models.
