About the Role
About You
- Doctor of Medicine or equivalent degree from an accredited institution.
- Current, full, and unrestricted medical license in a U.S. state or territory.
- Completed residency training in an accredited core specialty program.
- Proficiency in spoken and written English.
- Board-eligible or board certified in Internal Medicine/Family Medicine.
- Fellowship training in Geriatrics and/or Hospice & Palliative Medicine.
- Recent experience in geriatric transitional care programs.
- Experience in program development and multi-disciplinary quality improvement.
