- U.S. Citizenship.
- Degree of Doctor of Medicine or equivalent from an accredited institution.
- Current, full and unrestricted medical license in any U.S. State or territory.
- Completed residency training in an accredited core specialty program.
- Board Eligible/Board Certified by the American Board of Neurology.
