About the Role
As a Primary Care Physician, you will perform in-person comprehensive assessments, develop care plans, and oversee the care of participants in nursing homes and hospitals. You will collaborate with community-based physicians and participate in a shared phone call coverage schedule. Your responsibilities will include providing continuous medical coverage, maintaining care for nursing home participants, and participating in rotating night and weekend calls.
About You
Required:
- Board Certified or Board Eligible in Family Medicine or Internal Medicine.
- New York State medical license (or eligible for licensure).
- Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education.
- Doctorate Degree.
Preferred:
- Board Certification in Internal Medicine or Family Practice or Geriatrics.
