Conviva Senior Primary Care is seeking a Primary Care Physician to join their team in Port Orange, FL. The role involves evaluating and treating patients, focusing on outpatient medicine, continuity of care, health maintenance, and disease prevention.
About the Role
As a Primary Care Physician, you will be responsible for evaluating and treating center patients according to standards of care, coordinating patient services, participating in care team huddles, and ensuring compliance with medical documentation and policies. You will spend 100% of your time clinically focused on direct patient care, including administrative tasks related to patient care.
About You
Required:
- Current and unrestricted medical license or eligible to obtain a medical license in Florida.
- Graduate of accredited MD or DO program.
- Board Certification or Board Eligible in Family Medicine or Internal Medicine (Geriatric Medicine preferred).
- Active and unrestricted DEA license.
- Excellent verbal and written communication skills.
- Demonstrated high level of skill with interpersonal relationships and communications with colleagues/patients.
- Fully engaged in the concept of integrated team-based care.
- Willingness and ability to learn/adapt to practice in a value-based care setting.
- Superior patient/customer service skills.
- Basic computer skills, including email and EMR.
Preferred:
- Minimum of two to five years directly applicable experience preferred.
- New graduates will be considered.
- Experience managing Medicare Advantage panel of patients.
- Knowledge of Medicare guidelines and coverage.
- Knowledge of HEDIS quality indicators.
- Bilingual (English and Spanish) is a plus.
