About the Role
As a PRN Medical Social Worker, you will be responsible for assessing the psychosocial status of patients and families, providing counseling, crisis intervention, and education on advanced directives. You will collaborate with interdisciplinary teams to develop individualized care plans and maintain clinical records. The role also involves serving as a liaison between patients, families, and community agencies, and participating in quality assessment performance improvement activities.
About You
Required:
- A graduate of a master’s program in social work (MSW) accredited by the Council on Social Work Education or a bachelor’s program in social work.
- Minimum of one (1) year of experience in health care, preferably in hospice care.
- Good verbal and written communication skills.
- Current CPR Certification.
- Must be a licensed driver with an insured automobile.
Preferred:
- Experience in hospice care.
- Understanding of hospice philosophy and issues related to death and dying.
