About the Role
As a Home Health Physical Therapist, you will perform evaluations and treatments based on physician orders, establish treatment plans, maintain communication with the treatment team, and supervise assistants in patient care. You will work independently while having team support and contribute to the well-being of patients in their homes.
About You
Required:
- Minimum of Bachelor's Degree in Physical Therapy from an accredited program.
- At least 2 years of experience as a physical therapist.
- Valid license from the state of practice.
- Current CPR certification.
Preferred:
- Community/hospice experience.
- New physical therapy graduates may be considered in select markets.
