About the Role
As a Hospice IPC RN, you will be responsible for assessing, planning, implementing, and evaluating individualized care plans. You will coordinate with physicians and the interdisciplinary team to ensure compassionate care, maintain accurate documentation, and educate patients and caregivers. The position requires participation in 24/7 care coverage and adherence to nursing regulations.
About You
Required:
- Must be a licensed Registered Nurse in the state of practice.
- Previous experience in hospice, home health, medical-surgical, or population-specific care.
- Willingness to serve on-call and uphold the organization’s values.
- Must have a valid driver’s license and reliable vehicle.
Preferred:
- Associate degree preferred.
- CPR certification required in Georgia and California.
- Experience in geriatrics or pediatrics.
