About the Role

As a Registered Nurse (RN) – Home Care Initial Assessments, you will be responsible for performing comprehensive patient assessments, providing direct nursing care, and collaborating with an interdisciplinary care team to ensure high-quality, patient-centered care in clients' homes. Your daily tasks will include monitoring patient conditions, educating patients and caregivers, and documenting all assessments in the electronic medical record (EMR) system.

About You

Required:

Current Registered Nurse (RN) license in MA
Minimum 6 months to 1 year of nursing experience, preferably in home health, community health, or acute care settings with assessments.
CPR certification (BLS) required
Strong clinical and assessment skills
Excellent communication, time management, and organizational skills
Valid driver's license and reliable transportation
Ability to work independently and travel to patient homes

Preferred:

Experience with EMR systems
