About the Role
As a Registered Nurse Case Manager, you will be responsible for promoting patient-centered care by coordinating the plan of care for patients, managing their length of stay, and developing safe discharge plans in collaboration with a multidisciplinary team. Your duties will include performing comprehensive assessments, coordinating care transitions, and ensuring appropriate resource management.
About You
Required:
- Registered Nurse with current TX state license.
- Associate Degree in Nursing or Nursing diploma required.
- 3+ years of clinical hospital nursing experience required OR 2+ years experience in case management required.
Preferred:
- Bachelor's Degree in Nursing.
- Certification in case management.
- InterQual experience.
